Community-Based Health Care
The shift in the settings for health care delivery is a result of changes in federal legislation, tighter insurance
regulations, decreasing hospital revenues, and the development of alternative health care delivery systems. As a result of federal legislation passed in 1983 and 1997, hospitals and other health care providers are now reimbursed at a fixed rate for patients with the same diagnosis as defined by diagnosis-related groups. Under this system, hospitals and other health care providers can cut costs and earn income by carefully monitoring the types of services they provide and discharging patients as soon as possible. Consequently, patients are being discharged from acute care facilities to their homes or to residential or long-term facilities at much earlier stages of recovery than in the past. Don’t Just Do It, Be It!
Always document the care you deliver
Legal Requirements Your documentation must reflect the patient’s care status (condition/treatment) and include nursing interventions and outcomes of care. Documentation must demonstrate accountability of practice. Remember: The Clinical Record provides proof of the quality of care given to a patient and is admissible in court as a legal document. If it isn’t documented it didn’t happen.
The Process Clinical notes must meet the following criteria:
• they must be legible.
• they must be dated, timed and followed by author’s signature and designation.
• they must be a clearly identified signature. If your signature looks like spaghetti, print your name in brackets afterwards.
• each page must be labelled correctly,
• you must use only approved abbreviations as per hospital protocols. Read more…

Mengupas Realita, Membalut Duka Bangsa

Tolong koreksi! Sejak kecil, saya merasa sudah ‘didoktrin’ untuk menyintai lagu-lagu kebangsaan. Tentu saja saya belum mengerti apa maksud semua ini. Mulai duduk dari kelas satu Sekolah Dasar, setiap hari Senin, kecuali hujan deras atau liburan, saya selalu diajarin disiplin untuk mengikuti senam, upacara, mengibarkan bendera, menyanyikan lagu Indonesia Raya, mengheningkan cipta, mengenang jasa pahlawan yang telah gugur, mendengarkan petuah dan nasihat kepala sekolah dengan penuh khidmat, hingga yang namanya Pramuka.
Sejak Sekolah Dasar, Sekolah Menengah Pertama, Menengah Atas bahkan sesudah masuk Perguruan Tinggi sampai-sampai di tempat kerja pun, yang saya kira akan berakhir kegiatan rutin semacam upacara, ternyata masih berlangsung terus. Saya masih menyanyikan lagu-lagu kebangsaan tersebut…dan saya memang hapal betul…….”Mau dijadikan apa ya saya ini sebenarnya?” Begitu bisikan hati saya. Read more…
